Stop medication confirmation dialog with reason field Stop active medications when patients experience adverse effects, require dosage changes, or when switching to alternative treatments. The system requires documentation of the stop reason for clinical and compliance purposes.

Locate Stop Button

In the Active Medications table, find the medication you need to discontinue and click the Stop button in the Actions column.The stop action is immediately available for any active medication without additional permissions required.

Complete Stop Reason

Completed stop medication form with reason and notesIn the Stop Medication dialog:
  • Stop Reason (required): Provide the primary reason for discontinuation
  • Additional Notes (optional): Add clinical context, side effects, or next steps
The Stop Medication button remains disabled until you provide a stop reason. This ensures proper clinical documentation.

Confirm Medication Stop

Review the medication details shown in the confirmation message, then click Stop Medication to proceed.The action cannot be undone, but the medication history will be preserved in the Previously Active Medications section.

Verify Success

Success message and updated medication tablesAfter stopping the medication:
  • A success message appears: “Medication stopped successfully”
  • The medication is removed from the Active Medications table
  • The stopped medication appears in Previously Active Medications with your stop reason and timestamp
The medication history provides a complete audit trail of when medications were started, stopped, and why.
Common Stop Reasons:
  • Patient experiencing side effects
  • Switching to alternative medication
  • Treatment course completed
  • Patient request
  • Clinical review decision
Note: Once stopped, medications can be restarted by creating a new prescription rather than reactivating the previous entry.
